Observation Cards
One way to document a behavior based safety observation are Safety Cards. Performing safety observations are very easy and most effective when workers have a pocket size card to jot down their observation. With this card you are able to observe and understand the real safety related conditions and behaviors, and do something about it. Cards should follow the guideline below:
- Not to be used as disciplinary action, but as observation and discussion tool
- Used to track behaviors witnessed by employees
- Are anonymous
- Cards are used to document safe and unsafe behaviors
- Could also be used for documenting near misses
Things to keep in mind when writing your observation: pay attention to detail, and document precisely the behavor, what are you documenting? PPE, working conditions or hazards? fire or safety equipment? electrical hazards? driving hazards? It is always good to document the time… any evidence you can provide for the behavor?
